If you would prefer to have your information returned to the custody of Dr. Maia Gaither, MD and Greater Atlanta Integrative Pediatrics please reach out to Functional Genomic Analysis, see directions below.
“The transfer has to come directly from the parent from the email on file for each client. Please have them say "I, name of the parent, would like to transfer (name of child) to Dr. Maia Gaither's office portal" followed by their written name as a signature.”
